Claiming Supports
SIL is claimed weekly based on the regular SIL hours of support, typically it is your regular SIL funding specified divided by the number of weeks in your plan. Your Roster of Care will determine the ratio of support you will receive. These ratios are flexible and can be determined in consultation with you. Irregular SIL supports will be charged in addition to the weekly claim. Breakthru tracks the number of hours and ratios to ensure the appropriate charges apply.
Breakthru calculates the delivery and ratio of your support hours by utilising the NDIS SIL Roster of Care Tool. A copy of this will be provided by your Home and Living Service Manager within one month of receiving this schedule of supports
Vacancy Impacts
When a vacancy impacts shared support ratios, Breakthru will ensure you have the reasonable and necessary supports required to maintain your supported living arrangements.
Hospitalisation or other periods of absence (eg. holiday)
If
you are hospitalised or absent, Breakthru may continue to claim SIL supports to ensure continuity of services and less disruption for the other residents whom you share support with. It also assists to maintain staffing levels upon your discharge / return
Exiting – Notice Period
You agree to provide 6 weeks notice should you wish to cease SIL services with Breakthru. If you exit prior to the required 6 week notice period, Breakthru will claim SIL supports for the remaining notice period.
Breakthru may show the property to potential tenants during the notice period.
In the event of a clients passing, Breakthru may claim supports for a period of up to 28 days or 4 weeks at the specified weekly rate in the clients plan.
