This Schedule of Supports will form part of your Service Agreement with Breakthru.
The Schedule of Supports must be updated after each plan review or change to circumstances.

Home and Living Supports

Claiming Supports

SIL is claimed weekly based on the regular SIL hours of support, typically it is your regular SIL funding specified divided by the number of weeks in your plan. Your Roster of Care will determine the ratio of support you will receive. These ratios are flexible and can be determined in consultation with you. Irregular SIL supports will be charged in addition to the weekly claim. Breakthru tracks the number of hours and ratios to ensure the appropriate charges apply.  

Breakthru calculates the delivery and ratio of your support hours by utilising the NDIS SIL Roster of Care Tool. A copy of this will be provided by your Home and Living Service Manager within one month of receiving this schedule of supports

Vacancy Impacts

When a vacancy impacts shared support ratios, Breakthru will ensure you have the reasonable and necessary supports required to maintain your supported living arrangements.

Hospitalisation or other periods of absence (eg. holiday)

If you are hospitalised or absent, Breakthru may continue to claim SIL supports to ensure continuity of services and less disruption for the other residents whom you share support with. It also assists to maintain staffing levels upon your discharge / return

Exiting – Notice Period

You agree to provide 6 weeks notice should you wish to cease SIL services with Breakthru. If you exit prior to the required 6 week notice period, Breakthru will claim SIL supports for the remaining notice period.  Breakthru may show the property to potential tenants during the notice period. 

In the event of a clients passing, Breakthru may claim supports for a period of up to 28 days or 4 weeks at the  specified weekly rate in the clients plan.

Client acknowledgement

  • This Schedule of Supports must be read in conjunction with your Service Agreement

  • Breakthru are registered as a Temporary Transition Payment (TTP) provider. TTP rates will be applied where eligible 

  • Prices are subject to change during this agreement in line with NDIS Pricing Arrangements and will automatically be applied/increased 

  • Additional expenses (e.g., items not included as part of a customer’s funded supports) are the responsibility of the customer or nominee and are not included in the cost of supports above. E.g., entrance fees, event tickets, meals,etc 

  • Short Notice Cancellation charges will apply where notification of cancellation or changes to support is not received within 2 business days or  7 Days notice if you recieve Home and Living Supports.

  • Breakthru will claim for supports already delivered where the plan has been delayed/extended/reviewed or where the Service Agreement has not been returned within 14 days 

  • Where possible, service bookings  will be created at the category level to claim for supports delivered. Stated supports must have an item number

  • Breakthru may suspend services if you are not willing to pay the fees you have agreed to, in the timeframe outlined in the service agreement

  • Should either party require to end the Service Agreement or Schedule of Supports, we agree to give 14 days' written notice (or 30 days notice if you receive SIL)

  • To ensure continuity of services for current clients, Breakthru will continue to claim supports already delivered based on your previous schedule of supports where a plan has been extended, delayed or reviewed. If you're issued a new plan, Breakthru will require the funding details to provide a new schedule of supports. 

  • For new clients, Breakthru will claim for supports delivered from the start date of this schedule of supports.
